I love your ideas. I would love to provide the services you described in your scenarios. Unfortunately, I have a hard enough time getting paid under the current system. I have to fight insurance companies for appropriate traditional payments. Also patients don't want to dip into their HSA to even pay my copays. Then they want the rest (phone calls etc) for free. We have to change the publics attitudes as well as the insurance system.
