The feds have shown an unwillingness to approve state Medicaid and SCHIP waivers that extend coverage to children above 250% of the federal poverty level. Ohio was recently turned down for their plan to extend coverage up to 300% of FPL.
A state like Iowa, which is looking to expand coverage to all kids, would have a very difficult time getting such a plan approved with the current regime in charge of the federal Medicaid program. That could change if a Democrat is elected President, however.
