With a government run health care system, would the providers be contractors or employees? If they are contractors, where's the savings? If they are employees - can you picture doctors working for the same wages as teachers?
Actually, I think it is more accurate to say that various providers are confused about who their customers are right now. Network physicians work for the network, hospitals have been toadying to government since the 1930s. Insurers mostly work for employers and so on.
I really think that worrying about price transparancy before you shift control over funds to individuals is putting the cart before the horse and leaving a large amount of room for destructive government intervention in the name of ensuring transparancy--worthless reporting and so on.
Excellent points here on the market's ability to innovate when it is allowed to work. I would argue, however, that its not just proponents of government run health care who stifle consumer directed health care. Various stakeholders in health care talk the virtues of free enterprise and then work as hard as they can to fight it. Think of doctors and hospitals who won't post prices, insurance companies that use Medicare's fee and procedure schedules rather than negotiating rates, and HR departments in the 88% of US firms who offer one health plan selection.
