Top-down or bottom-up?

Mike Walden, economics professor at NC State University, lays out the two main approaches to health care reform, and pretty much everything else economic. Should dispersed consumers decide how to get care and what care to get in a bottom-up, adaptive system, or should a centralized planning agency decide who gets how much.

Walden's betting on consumers, but his own assumption that universal insurance coverage is needed points to the difficulties inherent in this debate.
